Medical Conditions/Action Plans

 

Chronic health conditions may be mild or intermittent, such as seasonal allergies, or life-threatening, as in anaphylaxis (allergic reactions).

Regardless of the severity, the school nurse will need updated medication information and healthcare

plans if the condition is life-threatening or interferes with the student’s ability to fully participate in the

educational process. Nurses will need to work with families to obtain necessary prescriptions, treatment

plans, and emergency action plans. Below are forms you may print and bring to your child's medical practitioner. Most offices

will fax your student's approprate Action Plans for management of their chronic conditions. The Fax Number is 302-672-1733
